• Addt'l Info: Mix of UAS and LCHSA Assessments, UAS certified, covering Brooklyn, Bronx, Manhattan and queens , 20-40 visits per month. *LCHSA and UAS assessments performed, mostly LCHSA Assessments. ?Home care experience required, UAS experience required, need to be UAS certified. Understands Geriatric population. ?They use the HHA Exchange System for Electronic Charting. ?An ongoing per-diem position ?15 to 20 Start of Care/Initial cases monthly with progressive visits (both Initial and Revisits) up to and maybe a little more of 50 visits a month. ?Training is unpaid, Remote training or in-office training if necessary. ?Office is in Brooklyn. ?LCHSA SOC assessments take an Hour (includes physical assessments + signatures) $88.50, LCHSA re assessment should only take 20 minutes $65, UAS Assessments $170. ?Short two hour training, self-explanatory computerized charting. ?Electronic Documentation, needs to be completed within the week latest. ?In person interviews are done

Minimum Requirements:

• Licensure + Education: A valid RN license in the state of practice, Certification: BCLS, ACLS, NRP, NIHSS, TNCC, & PALS according to the specialty assigned.
• Skills: Clinical Rotations: Minimum of 2 years of recent RN experience
• Submission Requirements: Resume, 2 References, Skills Checklist, License Verification, COVID Vaccine if requested by the facility
